No significant change ii} design or operation from that described <br />in items "I" A, B & C of the "FINDINGS" section is allowed, excel <br />for those changes which are required under the "CONDITIONS" - <br />portion of this permit. <br />2. The operator will immediately commence and thereafter maintain <br />a. -log of special occurrences. <br />3. Incoming records of weight or volumes to within 10% accuracy <br />must be kept by the operator for review by the San Joaquin - <br />Local Health District upon request. <br />4. A location of all accurate records pertaining to the station <br />operation must be provided to the San Joaquin Local Health <br />District•by August 15, 1978. <br />D. Provisions: <br />1. The following provisions must be met: <br />a. Identification signs shall be provided at each point of <br />access to the station indicating the name of the station <br />operator by August 1, 1978. <br />b. Wash facilities properly drained and hooked up to city <br />sewers must be provided at the station by September 1, 1978. <br />c. Approved safety helmets and respirators shall be provided <br />for employees operating equipment at the station by August <br />1, 1978. <br />2. This permit is subject to review by the San Joaquin -Local- Health - <br />District, and may be suspended, revoked or modified at any <br />time for sufficient cause. <br />E. Self -Monitoring Program: <br />1. The following items shall be monitored by the operator of this <br />facility or his agent and records shall be kept and made avail- <br />able to the enforcement agency upon request: <br />a. Quantity and types of wastes received at the station per <br />day and per week. <br />b. Quantity and types of wastes salvaged per month. <br />c. Number of vehicles utilizing the station per day and <br />per week. <br />
