Box 388 • Stockton, CA 95201-0388 �q ..• �P <br /> 209/468-3420 <br /> September 7, 1995 <br /> Gerald & Marilyn Mcdonald <br /> 10900 tokay Colony Rd. <br /> Lodi, CA, 95240 <br /> dol <br /> RE: Proposed "MGM Old English Mastiffs" <br /> 10900 Tokay Colony Rd. <br /> Lodi, CA, 95240 <br /> Dear Mr Mcdonald: <br /> This letter will indicate Health Department approval of drawings <br /> submitted for the proposed "MGM Old English Mastiffs" subject to <br /> the following conditions. <br /> 1) Animal food will be stored and fed in a manner which will not <br /> serve as harborage or breeding site for flies or rodents. <br /> Floors shall be durable, smooth and impervious to water, and be of <br /> an easily cleanable construction. <br /> You must keep this office informed of your construction progress <br /> and call for inspections (48 HOURS IN ADVANCE) when the facility is <br /> ready to open, all equipment installed and after thorough cleaning. <br /> Afee of $78 . 00 per hour may be assessed for any additional <br /> inspections, reinspections or consultations. <br /> Any discrepancy inadvertently overlooked in the plan check process <br /> must nevertheless be constructed or reconstructed to comply with <br /> all applicable codes. Inferior workmanship, equipment, or <br /> materials will not be accepted for the construction or operation of <br /> a food facility. No departure from the specifications outlined in <br /> this approval letter is authorized without prior written approval <br /> from this department. <br /> These plans shall become null and void if the work authorized is <br /> not commenced within 180 days from the date shown above, or if the <br /> work authorized by such plan approval is abandoned.
