Ash stockpiled prior to November, 1992 by Wadham Energy, Inc., Williams, Ca <br /> is prohibited at this site. <br /> 8. In the event ash is applied during any month, submit monthly ash dumping <br /> reports. Provide the amount of daily tonnage and number of trucks. <br /> 9. Obtain a written determination from RWQCB on whether a report of waste <br /> discharge and/or an application for waste discharge requirements will be <br /> required. In the event the RWQCB determines it has jurisdiction over the ash, <br /> the applicant must comply with all terms set forth by the RWQCB. <br /> 10. Provide a copy of the quarterly monitoring reports to EHD of the Wadham <br /> Energy, Inc. ash as required by the Department of Health Services letter of July <br /> 8, 1991. <br /> 11. Provide health and safety equipment to field workers who will be handling the <br /> ash operations at each site. <br /> 12. All conditions of this permit must be met prior to the beginning of operations. <br /> 13. Cover the existing stockpiled ash until Department of Toxics Substances <br /> Control makes a determination on whether the ash is a hazardous waste. If the <br /> ash is hazardous waste the applicant shall remove the ash within 45 days of <br /> the date of determination or as directed by the Department of Toxics <br /> Substances Control (DISC). If the ash is non-hazardous waste, the operator <br /> may apply for one time variance for application of this ash at this site. Variance <br /> application shall be made within 30 days of DISC date of determination, or the <br /> material shall be removed from the site within 45 days. <br />
