• A summary of work activities completed in the last week, and those expected to be performed in the next week. • A summary of deficiencies and/or defects, and resolutions. • Ongoing summary of changed and/or change orders to the work. • The signature of the QA/QC Manager. • On fourth week of each month the report will include a summary on on-site and third party laboratory test results. 6.3 Photographs Construction activities will be photographed. Photographs will include any significant problems encountered and corrective actions as will as document construction progress.The photographs will be identified by number,location,time,date,and photographer.The photographer should document the subject or the photograph,either on the back of the picture,or in a photograph log. One copy of the photographs and the negatives will be given to the Owner. 6.4 Design and Specification Changes Design and specification changes may be required during construction. Design and specification changes will only be made with written agreement of the Design Engineer and Owner. These changes will be made by change order to the contract. When change orders are issued,they will be prepared by the Construction Manager.The Construction Manager will distribute change orders for signature and execution to the required parties. 6.5 Final CQA Report At the completion of the project,the QA/QC Manager will submit a final CQA report. This report will document that the work has been performed in compliance with the Construction Drawings and Specifications. At a minimum, the report will contain: • A summary of all construction activities. • A summary of all laboratory and field test results. ® Sampling and testing location drawings • A description of significant construction problems and the resolution of these problems.
