I can close items : 101 for EPA ID , 110 for CERS <br /> Submittal , and 403 for open containers . <br /> Documentation provided for item 608 for not having bills of lading for forklift maintenance <br /> has some missing information . We need to know where the filters are being taken to and <br /> quantity . The forklift records also show that waste oil was removed but does not show where <br /> it is going . The forklift servicing company is required to leave a bill of lading ( or contain all <br /> required information on invoice ) when they transport drained used oil filters generated at <br /> your facility . If they have not been leaving the required information and are unable to <br /> provide this information , please ensure that going forward , they provide bills of lading for the <br /> oil filters . The following information is from the inspection report : <br /> REGULA TION GUIDANCE.• <br /> Persons generating, transporting, orreceiving used metal oil filters shall use a bill of lading to record <br /> the transfer of used oil filters. Bills of lading must indicate generator, transporter, and receiving <br /> companynames, addresses, telephone numbers, the quantityand size of used oil filter containers <br /> transferred, and the date of transfer. A copy of each bill of lading must be kept on the premises of the <br /> generator, transporter, and receiving facility where the used oil filters were handled. Copies of bills of <br /> lading shall be kept fora period of three years. <br /> In the attached pdf , there is more information on the last page under paragraph heading : <br /> " Transportation : Who and how ? " <br /> Please resubmit a corrective action statement for 608 telling us where ( name of receiving <br /> facility and address ) the filters are being taken to and quantity . Also include the used oil and <br /> where it is being taken to .
