San Joaquin County <br /> Environmental Health Department <br /> 1868 East Hazelton Avenue, Stockton, California 95205-6232 <br /> Telephone:(209)468-3420 Fax:(209)468-3433 Web:www.slgov.orq/ehd <br /> Large Quantity Hazardous Waste Generator Inspection Report <br /> Facility Name: Facility Address: Date: <br /> TARGET T0738 2800 NAGLEE RD, TRACY January 28, 2019 <br /> SUMMARY OF VIOLATIONS <br /> (CLASS I,CLASS II,or MINOR-Notice to Comply) <br /> Item# Remarks <br /> 118 CCR 66265.16(d) Failed to maintain complete personnel training records. <br /> Katie Copeland, the assistant manager, provided a documented titled"New Hazardous Materials+ Blood Borne <br /> Training,"which contained names and job titles of employees that either completed or had not completed the <br /> training(IMG 3821), however she could not provide employee hazardous waste training records for the last three <br /> years at the time of inspection. <br /> Employee training records shall include: <br /> 1. the job title for each position at the facility related to hazardous waste management, and the name of each <br /> employee filling each job; <br /> 2. written job description for each position listed above; <br /> 3. written description of the type and amount of both introductory and continuing training that will be given to <br /> each person filing positions listed above; <br /> 4. records that document that the training has been given to and completed by facility personnel. <br /> Training records on current employees shall be kept until closure of the facility. Training records on former <br /> employees shall be kept for at least three years from the date the employee last worked at the facility. <br /> Ensure that employee training is properly documented. Submit a copy of the employee hazardous waste training <br /> records for the last three years to the EHD. <br /> This is a repeat violation, Class Il. <br /> 403 CCR 66265.173 Failed to keep hazardous waste containers closed except when adding or removing hazardous <br /> waste. <br /> The following observations were made throughout the facility: <br /> -Three empty containers of Ecolab hand sanitizer and detergents were observed on the floor in the janitorial room <br /> near the facility restrooms(IMG 3818). These containers were not labeled as hazardous waste.According to Katie <br /> Copeland,the empty containers should have been processed to be disposed of as hazardous waste. Katie removed <br /> the empty containers from the janitorial room and took them to the facility's hazardous waste storage area to be <br /> processed. <br /> -Three boxes of Nicorette Lozenge, two of which expired January 2018 and one which expired April 2018, and one <br /> box of Up& Up nicotine gum which expired January 2019,were found for sale on an aisle shelf located in front of <br /> the pharmacy(IMG 3848-3853). Katie Copeland stated these boxes of nicotine-containing items should no longer <br /> be for sale since they were expired or almost expired. Katie removed the four boxes from the shelf and took the <br /> boxes to the hazardous waste storage area to be processed as hazardous waste. <br /> All hazardous waste shall be placed in closed containers and containers shall be closed at all times except when <br /> adding or removing waste. <br /> Immediately ensure all hazardous waste is contained in closed containers ensure all hazardous waste containers <br /> are closed when not adding or removing waste. <br /> This is a repeat violation, Class ll. <br /> FA0007036 PR0525971 SCO01 01/28/2019 <br /> EHD 22-02 Rev.09/22/16 Page 7 of 9 Large Quantity Hazardous Waste Generator OIR <br />
