Wilson Way <br /> Stockton, CA 95205 <br /> San Joaquin County Public Health Services, Environmental Health Division (PHS-EHD) has been contracted by <br /> California State Water Resources Control Board (SWRCB) to act as the local implementing agency to oversee <br /> investigation and remediation of leaking underground storage tanks (UST). <br /> Upon entry into this Local Oversight Program in March 1992, this site agreed to utilize the reporting format <br /> established in the LUFT manual and the Tri-Regional recommendations. Current corrective action regulations <br /> require the collection and analysis of data necessary to assess the nature and vertical and lateral extent of the <br /> release and to determine a cost-effective method of cleanup. (California Code of Regulations Title 23, Section <br /> 2725) <br /> This site has failed to supply a consistent year of quarterly monitoring reports and as of the date of this letter, <br /> has not submitted any reports for 1994. <br /> PHS-EHD hereby directs you to initiate quarterly groundwater monitoring and sampling immediately, <br /> submit reports by the 15th of the month following the quarter's end, and submit a workplan to define the <br /> lateral limits of the soil and groundwater contamination documented for this site by November 1, 1994. <br /> Failure to comply with directives issued by PHS-EHD will result in formal actions and could jeopardize <br /> eligibility to receive funds under the UST Cleanup Fund. <br /> Please direct your submittals, questions, and all requests for witnessed field activities to Michael Infurna, <br /> at (209) 468-3454. <br /> Donna Heran, REHS, Director <br /> Enviro ental Health Division <br /> Michael Infuma, Senior HS Diane M*inson, REHS <br /> Site Mitigation Unit Supervisor <br /> MI <br /> c: Applied Consultants - M.
