Wilson Way <br /> Stockton, CA 95215 <br /> San Joaquin County Public Health Services,Environmental Health Division (PHS/EHD) has <br /> reviewed the "Preliminary Investigation and Evaluation Report" (PIER) dated October 7, <br /> 1991 as submitted by ETS Environmental & Associates and has the following comments. <br /> It has been confirmed that the groundwater at this site is contaminated, but the extent of <br /> contamination has not been defined. Additional monitoring wells need to be installed to <br /> determine the lateral and vertical extent of contamination in the soil and the groundwater. <br /> Additional measurements for gradient determination and sampling of the existing wells <br /> should be done to determine placement of additional monitoring wells. <br /> Pursuant to Section 2652(d) of the Underground Storage Tank Regulations (California Code <br /> of Regulations, Title 23, Subchapter 16) a quarterly monitoring report shall be submitted to <br /> Public Health Services, Environmental Health Division and Central Valley Regional Water <br /> Quality Control Board (CVRWQCB) until the investigation and cleanup of the site is <br /> deemed adequate. Reports are due by the 15th of the month following the quarter's end <br /> and shall include: <br /> -status of investigation and cleanup activities including the results of all <br /> investigations completed to date; <br /> -proposed continuing or next phase of investigation; <br /> -method of cleanup proposed or implemented, schedules for the completion <br /> of the investigation of the site and remediation; <br /> -method and location of disposal of the released hazardous substance and any <br /> contaminated soil, groundwater or surface water; <br /> -manifest required for transport of hazardous substances; <br /> -monitoring well data including groundwater elevation, pH, conductivity, <br /> gradient; <br /> -analytical results of groundwater or soil sampling; and <br /> -site map showing the "zero line" of contamination, if known, and changes in <br /> the above measurements over the last quarter. <br /> A Division of bn Joaquin County Health care serv+ccs <br />
