U-Haul Facility No. 710050 Page 3 of 4 <br /> 2701 North Wilson Way December 13, 2013 <br /> Stockton, California 95215 <br /> • Wells that have laboratory data from the last groundwater sampling event that report <br /> non-detectable concentrations of contaminants of concern and with acceptable <br /> laboratory detection limits, and are not in areas of known or suspected soil <br /> contamination, may be destroyed by pressure grouting form the bottom upward, <br /> while leaving the well casing in place. The top three feet or more of the well shall be <br /> drilled out with an auger that is at least one foot larger in diameter than the auger <br /> originally utilized to install the well, and the soil around the cut-off casing removed to <br /> the depth of an additional six inches below the new `top' of the cut-off casing. The <br /> grout sealant shall spill out of the top of the well casing to form a mushroom cap seal <br /> with at least twelve inches of neat cement grout poured six inches above and six <br /> inches below the top of the cut-off casing. <br /> • All groundwater displaced to the surface during grout installation shall be pumped <br /> out of the excavation to allow for EHD inspection and verification that the well has <br /> been properly destroyed. Contaminated groundwater pumped out of the excavation <br /> shall be containerized for proper disposal. <br /> • EHD records indicate the presence of 11 `nested' wells [MW-1(A/B) through <br /> MW-6(A/B); MW-9(A/B); MW-10(B/C) through MW-12(B/C); and VE-1] installed <br /> using 6%-, 10'/4- and 12-inch diameter augers. The EHD has found destruction <br /> results of nested wells are best obtained when augers larger in diameter than the <br /> original augers are used and centered on the deepest casing (which is off-center in <br /> the original borehole). The auger needs to be sized sufficiently to reach out and <br /> remove all well casings, screens, filter packs, and grout sealant installed during <br /> construction. <br /> • This site has a number of wells located off-site in the City of Stockton right-of-way. <br /> Please submit a copy of the encroachment permit issued by the City of Stockton <br /> when submitting the EHD Well and Boring Permit Application form for these wells. <br /> • This site has a number of wells located on an adjacent parcel. Please submit a copy <br /> of the access agreement signed by the adjacent parcel owner when submitting the <br /> EHD Well and Boring Permit Application form for these wells. <br /> After the EHD has approved the well destruction scope of work proposal and the public <br /> participation comments have been properly addressed, you may submit an EHD Well and <br /> Boring Permit Application form along with a Master File Record form, encroachment permit <br /> issued by the City of Stockton, access agreement from the property owner where off-site <br /> wells are located, and appropriate fees for each parcel. The permit fee for the destruction of <br /> each well is $125, along with a $375 Inspection Activity fee per parcel. A review of the site <br /> records indicate that twenty-three monitoring wells, six vapor extraction wells and thirty-two <br /> air sparge wells currently are installed. A fee of $8,750.00 (61 x $125 + $375 x 3) will be <br /> required for the well destruction permits. (To determine the fees for nested wells, each <br /> nested well is considered as one well for permitting and fee purposes.) <br /> The EHD will issue a closure letter once the sixty-day public participation notification <br /> requirements and opportunity for public participation in the case closure decision process <br /> have been completed in accordance with the LTCP, all wells have been property destroyed <br /> and waste manifests have been submitted to this agency. <br />
