Wilson Way, Stockton,CA(Site Code: 1306) <br /> Addendum to Workplan for Destruction of Water Supply Well at Frontier Motel <br /> Dear Mr. Infurna: <br /> At your request, we are writing on behalf of Mr. John Ferraiolo to present you with an addendum <br /> to our July 1, 2005 Workplan for the destruction of the inactive water supply well located at the <br /> Frontier Motel, 2639 N. Wilson Way, Stockton, CA. In phone conversations, you requested <br /> additional detail and clarifications concerning the proposed well destruction. <br /> Workplan <br /> The inactive water supply well will be destroyed by perforating the casing and grouting to the <br /> surface with sand/cement slurry. The well destruction will be conducted by Water Well <br /> --. Technology, Inc. (C57 License No. 731133). <br /> L <br /> D Prior to perforating, down-hole equipment including the pump and piping will be removed and <br /> the total depth of the well will be measured. The wellbore will be video surveyed to document <br /> the condition of the casing. The well pad will be removed and a hole at least one foot larger in <br /> diameter then the original boring will be excavated around the casing to a depth of at least three <br /> feet below grade. The well casing will be cut off approximately six inches above the bottom of <br /> the excavation and removed. <br /> A 2-inch diameter tremie pipe will be used to fill the wellbore with a 10 sack sand/cement slurry. <br /> The well casing will be perforated under the wet slurry using 360-degree explosive booster <br /> charges initiated by detonating cord. The booster charges will placed at 10-foot intervals from 15 <br /> feet below grade to total depth. After detonation, additional grout will be pumped into the well <br /> as necessary to fill the casing to the top and allow for spill-over to form a mushroom cap. The <br /> excavation will subsequently be backfilled to grade. A blast mat will be used to confine any <br /> cement splashback. Water displaced from the wellbore will be contained in drums.
