Wilson Way <br /> Stockton, CA. 95205 <br /> The water provided by the drinking water well (well) at the Frontier Motel (motel) <br /> was impacted by from petroleum contamination as a result of the unauthorized <br /> releases from the underground storage tanks located at the 2662 and 2701 N. <br /> Wilson Way. San Joaquin County Environmental Health Department (EHD) <br /> required that this well be properly destroyed when the motel was hooked up to a <br /> municipal drinking water supply. By December 1 , 2004, the hook up to California <br /> Water Service was complete and the well at the motel was taken out of service. <br /> EHD has contacted your consultants numerous times about the well destruction. <br /> On July 5, 2005, Ground Zero Analysis (GZA) submitted a work plan to destroy <br /> the well by wire line explosions. EHD informed the consultants that modifications <br /> and clarifications were needed on the work plan. As of the date of this letter, <br /> EHD has not received modifications to the work plan. <br /> You are hereby directed to submit a work plan that proposes an adequate <br /> and cost-effective way to destroy the well. The work plan is to be <br /> submitted to EHD by August 19, 2005. A permit application form and $150 fee <br /> is to be submitted with the work plan. <br /> EHD will review the work plan and inform you if it is approved. The fieldwork is to <br /> be conducted by the C-57 licensed contractor within two weeks of permit <br /> approval. Please take steps now to work out a schedule with the driller. <br /> Responsible parties are required to be in compliance with directives and <br /> requirements issued by EHD in order to remain or become eligible to receive <br /> reimbursement from the State Water Resources Control Board (SWRCB) Clean <br /> Up Fund. By not completing the requirements of this water supply transfer, you <br /> are not in compliance with EHD requirements and the SWRCB will be notified. <br />
