Hazelton Avenue Stockton, CA 95205 Subject: Notice to Comply Kaiser Permanente 7373 West Lane Stockton, CA 95210 Dear Inspector Woldearegay, This letter is in response to the Underground Storage Tank Program Inspection Report dated June 25th, 2020 which was issued to the subject facility. Please be advised that the following issues have been addressed: CCR 2638(c), 2638(d) Failure to submit "Monitoring System Certification Form" within 30 days of completion of the test. OBSERVATION: Annual monitoring system certification and leak detector testing was performed on 6/6/2019 and a test report was submitted on 7/12/2019, 36 days late. REGULATION GUIDANCE: A copy of the test report must be submitted within 30 days of the tests. CORRECTIVE ACTION: Take all necessary precautions to ensure testing and submittal of test reports are performed in a timely manner. Corrective Action: The 6/6/19 Monitor Certification and Spill Container Testing results for the subject facility were emailed to the San Joaquin County agency test submittals inbox by Emily Tran of Belshire on Wednesday, 6/19/19. Please see the attached 6/19/19 email pdf. We respectfully request that this violation be rescinded.
