The incorrect and missing information included the following from the inspection reports: <br /> -The 7/24/19, 8/22/19, 10/17/19, 12/13/19, 01/10/20 and 2/7/20 DO reports left section V blank- The owner/operator <br /> failed to provide explanation of follow-up actions that will be taken for the statement provided by the DO in section <br /> III. <br /> -The 06/29/20 DO report left section VI blank-The OWNER/OPERATOR failed to provide a name, signature, and <br /> date within 48 hours of receiving the DO report under the ACKNOWLEDGMENT OF COMPLIANCE ISSUES <br /> section. <br /> -The 02/07/20 DO report did not include the name of the owner/operator. <br /> REGULATION GUIDANCE: During the visual inspection, the designated operator shall check that all required <br /> testing and maintenance for the UST system have been completed and shall check all required items on the report. <br /> CORRECTIVE ACTION: Ensure that designated operators performing visual inspections at this facility are including <br /> all of the required information on the reports. <br /> This is a minor violation. <br /> 211 CCR 2715(c)(2) No employee present that were trained in the operation and maintenance of the UST system by a <br /> DO. <br /> OBSERVATION: The designated operator employee training for employees was not current. <br /> REGULATION GUIDANCE: The designated operator shall train facility employees in the proper operation and <br /> maintenance of the UST system at least once every 12 months. The training shall be conducted before the <br /> individual performs the duties of a facility employees. The training shall include, but is not limited to: <br /> 1. Operation of the UST system in a manner consistent with the facility's best management practices <br /> 2. Employee's role with regard to monitoring equipment as specified in the facility's monitoring plan <br /> 3. Employee's role with regard to spills and overfills as specified in the facility's response plan <br /> 4. Name of the contact person(s)for emergencies and monitoring equipment alarms <br /> Initial training shall also include a site-specific practical demonstration that provides both sharing information and <br /> evaluating knowledge for responding to emergencies, leak detection, monitoring alarms, spills and overfills.
