30 S <br /> ITE SPECIFIC HEALTH &SAFETY RE UIREMENTS <br /> The following sections of the HASP present the site specific health &safety requirements for the site program <br /> 31 HEALTH &SAFETY TRAINING <br /> Each SHSO and Field Team Member shall have attended and successfully completed an initial 40-Hour Health and <br /> Safety course prior to performing field activities This course shall meet the requirements of 29 CFR 1910 120 <br /> Each Field Team Member shall have attended and successfully completed an 8-hoar Refresher Training course <br /> within one month of the anniversary month of their original 40-hour course Further,the SHSO shall have completed <br /> an 8-hour Supervisor/Field Team Leader training course <br /> Each SHSO and Field Team Member shall review a copy of the site HASP and attend the site health and safety <br /> meeting conducted at the site prior to the commencement of field activities <br /> 3 2 MEDICAL SURVEILLANCE <br /> Each SHSO and Field Team Member shall have completed a medical surveillance program prior to performing field <br /> activities Further,while on site,if a Field Team Member exhibits signs or symptoms indicating possible <br /> overexposure to hazardous substances or health hazards,or injuries or exposure above safe exposure levels,the <br /> possible overexposure shall be reported to the Field Team Leader or SHSO as soon as possible,and the Field Team <br /> Member shall undergo a medical examination as soon as possible <br /> 3 3 MONITORING PROCEDURES AND EQUIPMENT <br /> 1 Monitoring shall be performed where there may be a question of Field Team Member exposure to hazardous <br /> 11 substances Air monitoring shall be used to identify airborne levels of hazardous substance This monitoring shall be <br /> performed in order to assure proper selection of engineering controls,work practices and levels of protection <br /> f Upon initial site entry,representative air momtonng shall be conducted to identify hazardous conditions,such as <br /> i exposure over permissible exposure limits or published exposure levels,the presence of flammable atmospheres, or <br /> oxygen deficient environments <br /> Periodic monitoring shall be conducted when the possibility of a hazardous condition has developed since prior <br /> monitoring Situations in which additional monitoring shall be performed include <br /> • When work begins on a different portion of the site, <br /> When contaminants other than those previously identified are being handled, <br /> When a different type of operation is initiated, <br /> I When conditions have changed at the site <br /> During the drilling and soil sampling at the site,the air space around the drilling ng and breathing area will be <br /> monitored for airborne hazardous substances Monitoring activities will be continuous for the entire time drilling and <br /> soil sampling activities are conducted at the site If the permissible exposure limits or published exposure limits are <br /> exceeded during the monitoring program,then required controls and safety measures shall be implemented <br /> IMonitoring will be conducted using a photoionization detector and oxygen meter Each instrument will be properly <br /> calibrated prior to use <br />
