S0 JOA U 1 Environmental Health Department <br /> U ;� i ` I AMENDED <br /> 1/10/2020 <br /> Underground Storage Tank Program Inspection Report <br /> Facility Name: Facility Address: Date: <br /> VALLEY PACIFIC LODI PLANT&CARDLOCK 930 E VICTOR RD, LODI January 14, 2020 <br /> SUMMARY OF VIOLATIONS <br /> (CLASS I,CLASS II,or MINOR-Notice to Comply) <br /> Item# Remarks <br /> 209 CCR 2716(a-e) Failure to conduct the designated UST operator visual inspection at least once every 30 days. <br /> 1)OBSERVATION:The designated operator failed to check and note the correct test dates for Secondary <br /> Containment and Monitoring System Certification testing on the designated operator inspection reports. <br /> -Secondary Containment testing was incorrectly stated to be 1/18/2018,testing was conducted on 1/19/2018. <br /> -Monitoring System Certification was incorrectly stated to be 5/16/2019,testing was conducted on 1/4/2019. <br /> REGULATION GUIDANCE: During the inspection, the designated operator shall check that all required testing and <br /> maintenance for the UST system have been completed and document the dates they were done. <br /> CORRECTIVE ACTION: Ensure that designated operators performing inspections at this facility are including all of <br /> the required information on the reports. <br /> 2)OBSERVATION:The designated operator failed to check and note all required items on the visual inspection <br /> reports. <br /> -The"NA"in sections VII through XI were not fully explained on the inspection reports. <br /> REGULATION GUIDANCE: During the visual inspection,the designated operator shall check that all required <br /> testing and maintenance for the UST system have been completed and shall check all required items on the report. <br /> CORRECTIVE ACTION: Ensure that designated operators performing visual inspections at this facility are including <br /> all of the required information on the reports. <br /> This is a minor violation. <br /> 215 CCR 2712(1) Failure to retain a copy of the permit to operate at the facility. <br /> OBSERVATION:A current copy of the UST operating permit was not found on site. The most recent permit on site <br /> was dated 2018. <br /> REGULATION GUIDANCE:A copy of the UST operating permit and all attachments and conditions shall be <br /> retained at the facility. <br /> CORRECTIVE ACTION: Immediately obtain a copy of the current permit and retain it on site. <br /> This is a minor violation. <br /> Overall Inspection Comments: <br /> An inspection report was provided to the facility operator on the day of inspection. The report was amended by <br /> the EHD. The amended report replaces the initial report. <br /> Complete and submit a copy of the Return to Compliance Certification form to the EHD with a statement <br /> documenting the corrective actions that have been or will be taken for each violation, and any supporting <br /> paperwork, by 2/13/2020. <br /> Starting September 1, 2018, all in-office CERS help will be provided at EHD hourly rate ($152).
