The following is an itemized list of underground storage tank violations that have not been addressed for EI Dorado Gas & Mart as of December 24, 2020. Open violations from May 28, 2020 inspection Violation#210-No employee present that were trained in the operation and maintenance of the UST system byaDO. OBSERVATION: The designated operator employee training for Nabeel Naveed was not current. REGULATION GUIDANCE: The designated operator shall train facility employees in the proper operation and maintenance of the UST system at least once every 12 months. The training shall be conducted before the individual performs the duties of a facility employees.The training shall include, but is not limited to: 1. Operation of the UST system in a manner consistent with the facility's best management practices 2. Employee's role with regard to monitoring equipment as specified in the facility's monitoring plan 3. Employee's role with regard to spills and overfills as specified in the facility's response plan 4. Name of the contact person(s)for emergencies and monitoring equipment alarms Initial training shall also include a site-specific practical demonstration that provides both sharing information and evaluating knowledge for responding to emergencies, leak detection, monitoring alarms, spills and overfills. At least one of the facility employees present during operating hours shall have current training. CORRECTIVE ACTION: Ensure that employees have been trained by the designated operator, maintain the list on site, and submit a copy of the training records to the EHD. ❑ This violation was corrected ❑This violation will be corrected by(date): ❑ Supporting documents included Describe actions taken or will be taken to correct violation: Violation#304-Failure to construct,operate,and maintain primary containment as product-tight. OBSERVATION: Little amounts of liquid was found in UDC 3/4 , indicating a leak in the primary containment. REGULATION GUIDANCE:All primary containment for the UST system must be product tight.
