The location for proposed monitoring well MW4 appears to be only 15 feet from <br /> MW3, MW4 should be placed further away from MW3. The location for proposed <br /> MW5 should be between CPT3 and B6 as recommended in the February 1, 2006, <br /> EHD letter. <br /> 2. The installation of a monitoring well in the proposed MW6 location has not been <br /> justified and is not approved. <br /> 3. The installation of more than one observation vapor extraction well has not been <br /> justified and is not approved. <br /> 4. Submit two Environmental Health Department (EHD) well permit applications, two <br /> $383 permit fees and a City of Stockton encroachment permit.
