This work has not been completed to date. <br /> If the work is not completed within 14 days of the date of this letter, PHS/EHD may <br /> request formal enforcement action be taken by the San Joaquin County District <br /> Attorney's office and any reimbursement of costs for investigation and remediation by <br /> the State Water Resources Control Board Clean Up Fund maybe jeopardized. <br /> A well permit application and $89.00 permit fee should be submitted to PHS/EHD <br /> immediately. After the permit has been issued the work may be scheduled.
