I also sent a filled out one minus the contact approval tiles and signatures. The blank PDF form can be filled out but not saved. I suggest filling in the blank one with the information I provided and print it. If you try and save it saves a blank sheet. You can then send me the copy and I will sign and send you back the document for your records, or I can come up and sign. The form and directions/terms are included. I am also available to meet or talk to the person or the persons if they have questions. The POD allows certified collectors like us to act in behalf of a local government... Because this Recyclers records, servers, and documents have been destroyed and unavailable we are seeking your help so we can use the POD to capture reimbursement from CalRecycle. The POD allows us to just record CRT and Flat Screen numbers and weights not individual names. Without POD approval we cannot claim the material from the State. This would help us recover the recycling costs for this material stream which is only a small part of the clean out.. We believe there is about 25,0001b of CRT devices and about 190,0001b of flat screens. We will be sending this material to an authorized Recycler or holding on to it to process ourselves once our re issuing Recycling permit is issued. (it is pending DTSC audit and blessing). THIS POD IS FOR A ONE TIME ONE SITE CLEANOUT OF RESIDENTIAL COLLECTED E- WASTE. THE POD WILL NOT BE USED FOR ANY OTHER PURPOSE You can be rest assured that all e-waste collected SB20 and other is being processed to the highest environmental standards since we are R2 Certified. We believe this is this POD will insure the health and safety of the people and City of Stockton while insuring residential material collected from handlers and collectors is processed to State standards and ethic responsibilities.
