Weber Street, Third Floor <br /> Stockton, California 95202 <br /> Subject: Work Plan for Site Investigation <br /> 1108 West Tokay Street, Lodi, California <br /> Dear Ms. Lagorio: <br /> At the request of Wendell Youngsma of SBA Business Loan Center for Bank of Lodi, <br /> regarding the dental office located at 1108 West Tokay Street in Lodi, California, Lush <br /> Geosciences Inc. has prepared this Work Plan to perform a soil and groundwater investigation at <br /> the site. The scope of work describes a proposed subsurface environmental investigation to <br /> evaluate whether soil and/or groundwater contamination has occurred at the site due to offsite <br /> activities. All work performed will be in compliance of San Joaquin County Public Health <br /> Services, Environmental Health Division and Tri-Regional guidelines. <br /> INTRODUCTION <br /> This Work Plan presents proposed methods for performing a soil and groundwater <br /> investigation at the site. The site is an operating dental office. <br /> SCOPE OF WORK <br /> We propose a subsurface environmental investigation to evaluate whether contamination <br /> has occurred at the site.
