SAN J OA U I Environmental Health Department <br /> COUNTY <br /> — <br /> Hazardous Materials Program Inspection Report <br /> Facility Name: Facility Address: Date: <br /> Shepard Bros. - Stockton 4407 Giannecchini Ln, Stockton July 10, 2020 <br /> SUMMARY OF VIOLATIONS <br /> (CLASS I,CLASS II,or MINOR-Notice to Comply) <br /> Item# Remarks <br /> 8 HSC 25505(a)(4) Failed to submit a training program or program is not reasonable or adequate for facility. <br /> An Emergency Response Plan was submitted in lieu of an Employee Training Plan. Training programs for all <br /> employees and annual training, including refresher courses, shall include familiarity with the facility's Emergency <br /> response plans and procedures in the event of a release or threatened release of a hazardous material. Training <br /> shall cover, but not limited to, all of the following: <br /> (A) Immediate notification contacts to the appropriate local emergency response personnel and to the unified <br /> program agency. <br /> (B) Procedures for the mitigation of a release or threatened release to minimize any potential harm or damage to <br /> persons, property, or the environment. <br /> (C) Evacuation plans and procedures, including immediate notice, for the business site. <br /> These training programs may take into consideration of each employee. Immediately prepare or update the facility's <br /> training program, log into the California Environmental Reporting System (CERS) at http://cers.calepa.ca.gov/, <br /> upload the correct or updated information, and submit to the EHD for approval. <br /> This is a repeat violation, Class ll. <br /> 10 HSC 25508.1(a)-(f) Failed to submit revisions to business plan within 30 days of substantial change in operations. <br /> During the inspection, ten 8 gallon and two smaller propane cylinder were found onsite and the updated information <br /> has not been reported. Within 30 days of one the following events, business shall electronically update the <br /> information submitted to the California Environmental Reporting System (CERS): <br /> -A 100 percent or more increase in the quantity of a previously disclosed material. <br /> -Any handling of a previously undisclosed hazardous materials <br /> -A change of business address, business ownership, or business name. <br /> Immediately log into the California Environmental Reporting System (CERS) at http://cers.calepa.ca.gov/, enter the <br /> correct or updated information, and submit to the EHD for approval. <br /> This is a Class II violation. <br /> FA0015645 PR0523312 SCO01 07/10/2020 <br /> EHD 19-01 Rev.9/20/2019 Page 3 of 4 Hazardous Materials Program OIR <br /> 1868 E.
