The following is an itemized list of hazardous waste violations that have not been <br /> addressed for E & A TIRE & AUTO REPAIR as of October'17, 2019. <br /> Open violations from August 15, 2019 inspection <br /> Violation #113-Failed to keep copies of consolidated manifesting receipts for three years. <br /> Copies of hazardous waste disposal records for August 2016 through December 2018 were not found on site. <br /> Hazardous waste generators shall retain copies of all manifests signed off by the disposal facility and all receipts <br /> used in a consolidated manifesting procedure on site for three years and have them readily available for review. <br /> Immediately locate a copy of all manifests and receipts for the last three years, maintain them on site, and submit <br /> copies to the EHD. <br /> ❑ This violation was corrected ❑ This violation will be corrected by (date): <br /> ❑ Supporting documents included <br /> Describe actions taken or will be taken to correct violation: <br /> Violation #303 -Failed to maintain all communication or alarm systems, spill control, or decontamination <br /> equipment. <br /> The secondary containment pallet for the used oil was observed with approximately 2 inches of used oil in it. Per <br /> Adan Cigarroa (Owner) the punctured filters are drained into the secondary containment pallet and it is pumped out <br /> when the oil is picked up. Several punctured metal oil filters were observed draining into the secondary containment <br /> pallet at the time of the inspection. All facility communications or alarm systems, fire protection equipment, spill <br /> control equipment, and decontamination equipment, where required, shall be tested and maintained as necessary to <br /> assure its proper operation in time of emergency. Immediately begin this practice. <br /> ❑ This violation was corrected ❑ This violation will be corrected by (date): <br /> ❑ Supporting documents included <br /> Describe actions taken or will be taken to correct violation: <br /> Violation #505 -Failed to completely label stationary hazardous waste tanks. <br /> One white plastic used oil tank was observed with the hazardous waste label on the wall. The hazardous waste <br /> label was missing an accumulation start date. All hazardous waste tanks shall be marked with the words <br /> "Hazardous Waste" and the accumulation start date. Immediately ensure that all hazardous waste tanks are <br /> marked with the required information. <br /> ❑ This violation was corrected ❑ This violation will be corrected by (date): <br /> ❑ Supporting documents included <br /> Describe actions taken or will be taken to correct violation: <br /> Page 1 of 2 <br />
