A J OA U I Environmental Health Department <br /> COUNTY <br /> Underground Storage Tank Program Inspection Report <br /> Facility Name: Facility Address: Date: <br /> 76 EXPRESS TIGER NO 1 5777 S FRENCH CAMP RD, STOCKTON January 14, 2020 <br /> SUMMARY OF VIOLATIONS <br /> (CLASS I,CLASS II,or MINOR-Notice to Comply) <br /> Item# Remarks <br /> 121 CCR 2712(f) Failure to implement corrections and submit written response within 30 days of receiving the report. <br /> OBSERVATION: An inspection was last done on 01/15/2019 and an inspection report was issued identifying <br /> information to be submitted to bring this site into compliance. This information was required to be submitted by <br /> 05/29/2019. This information has not been received resulting in a non-compliant status for this facility. <br /> REGULATION GUIDANCE: An operator that receives an inspection report shall have 30 days to implement the <br /> corrections specified in the inspection report and comply with permit conditions. The corrective action shall include <br /> all of the recommendations made in the report. The operator shall submit a written response that includes a <br /> statement documenting corrective actions taken or proposing corrective actions which will be taken. <br /> CORRECTIVE ACTION: Ensure that a written response documenting corrective actions taken or proposed is <br /> submitted and all corrections implemented within 30 days of receiving an inspection report. <br /> This is a repeat violation, Class ll. <br /> 124 CCR 2637.2(d&e), 2665(a&b)"Overfill Prevention Equipment Inspection Report Form" not submitted within 30 days <br /> of inspection. <br /> OBSERVATION: Overfill prevention testing was performed on 1/15/2019 and it appeared to contain inaccurate <br /> percent capacities calculated based on a linear calculation instead of using the tank chart volumes <br /> REGULATION GUIDANCE: An updated test report to include accurate calculations percentages has not been <br /> submitted to the EHD or the California Environmental Reporting System (CERS). A copy of the 'overfill prevention <br /> Testing Report Form' must be submitted within 30 days of the test. <br /> CORRECTIVE ACTION: Immediately provide this report to the EHD. <br /> This is a minor violation. <br /> 201 CCR 2712(b)(1-2) Failure to maintain monitoring, maintenance, and follow up action records. <br /> OBSERVATION: Maintenance and monitoring records for the last three years were not found on site. <br /> - Records of water removal from sumps in response to alarms were not found on site. <br /> -The 1/21/19 designated operator inspection report stated a fuel release was found in dispensers 17, 19, and 20 <br /> from a piping joint and stated this was addressed by Elite IV. No work order was found for this leak. <br /> REGULATION GUIDANCE: These records shall be maintained on site for at least three years. Monitoring records <br /> include: (1) date and time of all monitoring or sampling; (2) monitoring equipment calibration and maintenance <br /> records; (3) results of any visual observations; (4) results of sample analysis performed a lab or in the field; (5) logs <br /> of all readings of gauges or other monitoring equipment, ground water elevations, or other test results; (6) results of <br /> inventory readings and reconciliations. <br /> CORRECTIVE ACTION: Immediately locate and maintain all missing maintenance and monitoring records for the <br /> last three years on site and submit copies to the EHD. <br /> This is a minor violation. <br /> FA0006977 PR0505746 SCO01 01/14/2020 <br /> EHD 23-01 Rev.9/20/2019 Page 6 of 9 Underground Storage Tank Program OIR <br /> 1868 E.
