3 Installed three dual completion vapor extraction/air sparge wells (VEAS-1 through VERS-3) on-site <br /> for the purposes of conducting pilot study activities The two foot air sparge point was installed to a <br /> depth of approximately 65 feet bgs, with the corresponding vapor extraction well set at <br /> approximately 50 feet bgs, and screened from approximately 15 to 50 feet bgs in VEAS-1 and <br /> VEAS-3 VEAS-2 was screened from approximately 10 to 50 feet bgs <br /> The scope of work included preparation of a Community Health and Safety Plan, site reconnaissance, <br /> permitting, well drilling, organic vapor monitoring, soil sampling (for lithologic logging, analytical testing <br /> and waste characterization purposes), well surveying, groundwater gauging and sampling, analytical <br /> chemistry, data interpretation, and report preparation Each part is described below <br /> 5 1 Community Health and Safety Plan <br /> ATC has established a Safety and Health Program (SHP) to ensure the personal health and safety of all <br /> ATC employees The SHP defines safety practices and procedures to be instituted in all ATC work <br />' places, as applicable The program meets, and often exceeds, the requirements promulgated by the <br /> Occupational Safety and Health Act (OSHA) As part of the SHP, all ATC personnel are appropriately <br />' trained and under a Medical Surveillance Program in accordance with OSHA 40 CFR 1910 120 <br /> ATC's primary mechanism to ensure employee, environmental, and public safety at the project site was <br /> ' the Community Health and Safety Plan (CHASP) ATC prepared, approved and implemented a CHASP <br /> for this project All individuals working under the purview of ATC were required to read and sign the <br /> CHASP to acknowledge their understanding of the information contained in it The CHASP was site- <br /> specific and task-specific, describing hazardous conditions that may be encountered and prescribing the <br /> necessary safety protocols to protect employees from these hazards The CHASP was reviewed by the <br /> ' project management team and then reviewed/approved for field use by the Site Safety and Health Officer <br /> The CHASP was implemented and enforced on site by the assigned Site Safety and Health Officer ATC <br /> prepared a comprehensive CHASP based on the scope of work and the potential hazards described in the <br /> ' ATC work plan dated December 21, 2001 <br /> The CHASP identified the following roles and responsibilities of key site personnel, hazard analysis for <br /> all chemical, physical, and physiochemical hazards anticipated, a personnel protection plan, site safety <br /> procedures for specific site operations, (e g, drilling, etc ), a decontamination plan, and an emergency <br /> ' response/contingency plan The CHASP specifies levels of protection for site personnel on a task-specific <br /> basis As with any project of this magnitude, there are inevitable encounters with a variety of physical <br /> hazards ranging from simple housekeeping to temperature extremes ATC provided continual evaluation <br /> ' of potentially hazardous conditions, and as the project progressed additional safety protocols to protect <br /> site personnel were implemented as needed <br /> EZ 100966ASARrpt ATC Associates Inc <br /> ' <br /> \\S at)Diego1EZ Serve11009661Reports1100966SAR001 rpt ATC Project No 43 25827 0026 <br /> ' 6 <br />
