The following manifest numbers were found without a signed off copy from the destination facility: -2019 uniform Manifest were all missing a signed off copy from destination facility. -2018 uniform manifest 017696911JJK(8-1-18)and 017696912JJK(8-1-19)both missing signed off copy from destination facility. -2017 uniform manifest 016774117JJK(9-22-17)and 018236505JJK(11-14-17)both missing signed off copy from destination facility. Hazardous waste generators shall retain copies of all manifests signed off by the destination facility on site for three years and have them readily available for review. If the generator did not receive a copy of the manifest with the handwritten signature of the owner or operator of the facility to which the generator's waste was submitted within 60 days of the date the waste was accepted by the initial transporter, the generator shall submit a legible copy of the missing manifest,with some indication that the generator has not received confirmation of delivery. This information shall be submitted to: DTSC Report Repository Generator Information Services Section P.O. Box 806 Sacramento, CA 95812-0806 Immediately locate a copy of the missing manifests or prepare and submit the required information to DTSC. Submit to the EHD a copy of the signed off copy of the manifest or a copy of the information submitted to DTSC. This is a repeat violation, Class II. 301 CCR 66262.34(d)(2) Facility not maintained to minimize the release of a hazardous waste. -Approximately 10-15 yards of oily residue/liquid and used soap observed on the ground surrounding the approximately 200 gallon used oil tank and soap storage area outside the shop. -Oily residue/liquid observed leaking out of hose connected to approximately 100 gallon diesel fuel tank inside the shop. Facilities shall be maintained and operated to minimize the possibility of a fire, explosion, or release of hazardous waste to air, soil, or surface water which could threaten human health or the environment. Immediately clean spill , and manage according to Title 22 hazardous waste regulations. Submit a statement and disposal receipts for spill to EHD. This is a repeat violation, Class II.
