SAN JDANIN <br />COUNTY <br />Environmental Health Department <br />Small Quantity Hazardous Waste Generator Inspection Report <br />Facility Name: <br />Facility Address: <br />Date: <br />DOLLAR GENERAL #13632 <br />2907 E WATERLOO RD STOCKTON <br />July 29 2019 <br />SUMMARY OF VIOLATIONS <br />(CLASS I, CLASS II, or MINOR - Notice to Comply) <br />Item # <br />Remarks <br />106 <br />CCR 66262.34(d)(2) Failed to train employees on waste handling and emergency procedures. <br />Based on the types of violations observed, the facility employees are not trained on proper hazardous waste (HW) <br />handling or the training provided has been inadequate. <br />- When asked how employee's determine which HW tote a waste item should be placed into, Oscar Arvayo, Store <br />Manager, explained that when a waste item is scanned, the hand-held scanner will indicate which HW tote the item <br />should be placed into. Oscar stated that if the hand-held scanner does not indicate which HW tote an item belongs in, <br />employees refer to the Dollar General (DG) Environmental Compliance poster (Poster) and guidance documents found <br />posted next to the HW storage area, as well as the labels on the hazardous waste totes. Oscar stated that if he is <br />still not able to determine if a waste item is hazardous or which hazardous waste tote the waste item should be <br />placed in, he will contact the Loss Prevention manager. Oscar was not aware of the appropriate person or phone <br />number to call in case he or another employee had questions about an item's hazardous waste determination. (IMG <br />7745-6,7748) <br />- Oscar stated that a Hazardous Waste Storage Weekly Checklist form is completed each time he conducts the <br />weekly hazardous waste storage inspection. At the time of inspection, only two checklist forms were found completed <br />for July 2019, two were found completed for June 2019, none were found for May 2019, and two were found completed <br />for April 2019. Oscar stated that he may have been off of work during the weeks that were missing a Hazardous <br />Waste Storage Weekly Checklist, and thus the weekly inspections were not conducted. (IMG 7789-94) <br />- Three containers of TRP Pink Eye Relief eye drops were found in an open tote labeled "Recall." A Dollar General <br />"stop sales notification" document, also found inside the tote, was dated 7/17/2019 and stated "all inventory of the <br />TRP Pink Eye Relief' should be placed in a zip -lock bag and in the Toxic hazardous waste tote. Oscar placed the <br />three containers of eye relief in a bag and into the Toxic HW tote at the time of inspection. (IMG 7766-8) <br />- Oscar provided a list of current store employees. Oscar stated that Adri Delsied is on medical leave and needs to <br />complete the hazardous waste training, and Ruben Silva is a new employee and is going to complete the hazardous <br />waste training shortly. <br />The generator must ensure that all employees who handle hazardous waste are thoroughly familiar with proper waste <br />handling and emergency procedures. <br />Provide proof of re-training and training materials to the EHD for employees whose responsibilities include hazardous <br />waste. <br />This is a Class II violation. <br />FA0002093 PRO537154 SCO01 07/29/2019 <br />EHD 22-01 Rev. 09/27/2018 Page 6 of 11 Small Quantity Hazardous Waste Generator OIR <br />1868 E.
