The following is an itemized list of underground storage tank violations that have not been addressed for LODI OIL INC ARCO #83680 as of May 06 , 2020 . Open violations from May 09, 2019 inspection Violation #209 - Failure to conduct the designated UST operator visual inspection at least once every 30 days . The designated operator failed to check and note all required items on the visual inspection reports . The incorrect and missing information included the following from the inspection reports : -Section V was filled out by the designated operator and did not include the corrective actions that will be taken by the owner/operator. - Incorrect monitoring system certification and spill container test dates were listed . -Section V was left blank . -Reports were unsigned by the owner/operator . -The designated operators , including Brian Roth , are indicating that they are looking inside fill sumps ; Mr. Roth stated that fill sumps could not be opened during the testing , have never been opened by him , and appear as if they have not been opened in over a year. -The N and NA answers were not included in section III. -CERS ID was left blank . -The question regarding UDC sensor locations was left blank . -The question regarding sump sensor locations was left blank . -Overfill test information was left blank . -The reports incorrectly mark that all follow up to previously reported issues were completed and list repeat issues in Section III . During the visual inspection , the designated operator shall check that all required testing and maintenance for the UST system have been completed and shall check all required items on the report. Ensure that designated operators performing visual inspections at this facility are including all of the required information on the reports . The designated operator failed to document all the alarms from the attached alarm histories on the designated operator inspection reports and failed to check that they were responded to appropriately. The missing alarms include : -L2 ( UDC 3/4) fuel alarms on 2/26/19 and 1 /16/19 During the inspection , the designated operator shall review the alarm history since the previous inspection , check that each alarm was documented and responded to appropriately, and attach a copy of the alarm history with documentation taken in response to any alarms to the report . Ensure that designated operators performing inspections at this facility are including all of the required information on the reports . A designated operator inspection for the following months was performed more than 30 days : -Conducted 11 /9/18 , next conducted 12/10/ 18 (31 days) -Conducted 12/10/18 , next conducted 1 /16/ 19 ( 37 days) -Conducted 3/5/19 , next conducted 4/5/ 19 (31 days) The designated operator visual inspections shall be conducted within 30 days or less of the previous inspection .
