Shelly provided the most current record of training for current store <br /> employees,which shows that at least six employees,five of which are store managers,were last trained in <br /> September 2017. <br /> The generator must ensure that all employees who handle hazardous waste are thoroughly familiar with proper <br /> waste handling and emergency procedures. <br /> Provide proof of re-training and training materials to the EHD for all employees whose responsibilities include <br /> hazardous waste. <br /> This is a Class II violation. <br /> 109 CCR 66262.23(a)(4) Failed to send generator manifest copies to DTSC within 30 days. <br /> According to Shelly Felix, store manager,the facility does not mail manifests to DTSC. Generator copies of the <br /> following manifest numbers were not sent to DTSC within 30 days of shipment of hazardous waste: <br /> -008570581 FILE (8/6/2016)(IMG 5283-4) <br /> -011410896FLE (11/3/2018)(IMG 5281) <br /> -011410897FLE (11/3/2018)(IMG 5282) <br /> The generator manifest copy shall be submitted to DTSC for every shipment on a manifest when California is either <br /> the generator state or the destination or consignment state. The generator manifest copy shall be mailed to: <br /> DTSC Generator Manifests <br /> P.O.
