TRAINING ACKNOWLEDGEMENT SIGN-IN SHEET <br /> Melissa&Doug Locations: Mountain House,Perlman,Luce, and Schulte Locations <br /> Training Topic/Title: Emergency Action Plan and Hazardous Materials Plan <br /> Date of Training: 2/27/2020 <br /> Training Details: <br /> • In the event of an emergency you will be alerted by a verbal announcement, sounding of an <br /> alarm, and emergency signal air horn. <br /> • Please walk safely to the nearest exit and leave the building. <br /> • Portable fire extinguishers are available in the workplace for employee use. <br /> • No employees or temps are assigned to perform medical or rescue duties during emergency <br /> evacuation situations. <br /> • During an emergency evacuation, all individuals should report to the safe meeting area across <br /> from the guard shack in the back parking lot. Management will take attendance. <br /> • In the event of a leak of hazardous materials such as auto floor cleaning solution,please contact <br /> a manager immediately. The manager will use the absorbents in the designated spill kit and <br /> personal protection equipment to clean up the leak. <br /> • In the event of a propane leak,please immediately do the following: park the forklift and contact <br /> a manager. The manager will remove the propane tank from the facility and notify the supplier <br /> of the propane tank. <br /> • In the event of a battery acid leak,please contact a manager immediately.
