Syr' <br /> If11'IJ O `A Q 1 I "II1VI Environmental Health Department <br /> r111 Y - <br /> Small Quantity Hazardous Waste Generator Inspection Report <br /> Facility Name: Facility Address: Date: <br /> Dollar General#13568 560 N Main St, Manteca Julv 31 2019 <br /> SUMMARY OF VIOLATIONS <br /> (CLASS I,CLASS 11,or MINOR-Notice to Comply) <br /> Item# Remarks <br /> 405 CCR 66262.34(d)(2) Failed to manage incompatible wastes in containers properly. <br /> The incompatible waste items listed below were found bagged and inside a tote labeled Toxic HW(IMG 7971, <br /> 7979-84): <br /> -A Revlon Color Silk permanent hair color kit,which contained a container of ammonia-free colorant, a container of <br /> cream color developer, and a package of after-color conditioner. Hydrogen peroxide is listed as the second ingredient <br /> in the developer.According to the PSC HW guidance poster and documents, items containing hydrogen peroxide are <br /> to be placed in the Oxidizer HW tote. The hazardous properties of the ammonia-free colorant and the after-color <br /> conditioner are unknown. <br /> -A Broadway Colors Quick Dry nail polish,which was labeled as flammable.According to the PSC HW guidance <br /> poster and documents, flammable items are to be placed in the Flammable HW tote. <br /> Incompatible wastes shall not be placed in the same container, or in an unwashed container that previously held an <br /> incompatible waste or material. A container holding a hazardous waste that is incompatible with any waste or other <br /> materials transferred or stored nearby in other containers, piles, open tanks, or surface impoundments shall be <br /> separated from the other materials or protected from them by means of a dike, berm,wall, or other device. <br /> Immediately transfer or separate the oxidizer and flammable hazardous waste to other containers or separate the <br /> containers. <br /> This was corrected on site. <br /> This is a Class II violation. <br /> Overall Inspection Comments: <br /> Notes: <br /> -An unlabeled container of unknown liquid, which appeared to be used motor oil, was found on the ground <br /> outside of the Dollar General dumpster enclosure. Steve Bohnak, District Manager, indicated the container was <br /> likely left there illegally by an unknown individual. Immediately make a hazardous waste determination for this <br /> container and manage per Title 22 hazardous waste regulations. Submit a statement and supporting <br /> documentation explaining how this waste was managed. <br /> -A pack of C2 Energizer Max+ Power Seal alkaline batteries were found in the Universal Waste tote. The <br /> scanner stated the batteries should be placed in the Universal Waste tote,while the PSC HW guidance poster <br /> and documents indicate that alkaline household batteries should be placed in the Alkaline HW tote.
