`\++ ■jnN A M U(N Environmental Health Department
COUNTY—
Small
OU NTY
Small Quantity Hazardous Waste Generator Inspection Report
Facility Name: Facility Address: Date:
Dollar General#13568 560 N Main St Manteca Julv 31 2019
SUMMARY OF VIOLATIONS
(CLASS I,CLASS II,or MINOR-Notice to Comply)
Item# Remarks
103 CCR 66262.40(c) Failed to retain hazardous waste determination including waste analysis on site for 3 years.
When asked how employees determine which of the differently categorized hazardous waste(HW)totes a waste item
belongs in,Jackie Nash,Assistant Store Manager, said each item is scanned using a hand-held scanner, which will
sometimes state which HW tote the item belongs in.Jackie indicated that when the scanner does not specify which
HW tote an item belongs in, employees will read the item's label and ingredients, as well as the PSC HW guidance
poster and documents, which provide product examples and key words for each of the HW categories,to determine
which HW tote an item belongs in(see photos IMG 7952-3, 7988-9).When asked what employees do if they are still
unsure about which HW tote a waste item belongs in, Jackie did not seem aware to call the store maintenance hotline
phone number listed at the bottom of the PSC HW guidance documents.Additionally,Jackie and Steve Bohnak,
District Manager, did not seem aware of Dollar General's(DG)Respond ticketing system to submit a notification to
DG Corporate regarding the scanning system not providing a HW category for a particular waste item.
The waste items listed below were observed in the HW totes, but at the time of inspection it could not be determined
how the HW determinations for each of these items were made. The hand-held scanner did not specify which HW tote
the items should be placed in, and the PSC guidance poster and documents did not list these types of items:
1. The waste item listed below was found bagged and inside a tote labeled Toxic HW(IMG 7937, 7940-1, 7988):
-A tube of all purpose Krazy Glue. Steve Bohnak, District Manager, stated that he believed many adhesives or glues
were flammable.A tube of The Original Super Glue was found in the HW tote for Flammable HW(IMG 7964, 7967-8).
Also the PSC HW guidance document for Flammable HW pictured a tube of adhesive called Goop.
2. The waste items listed below were found bagged and inside a tote labeled Oxidizer HW(IMG 7959-61):
-A leaking container of Arm&Hammer Laundry Detergent Plus Oxi Clean with Fade Defense Sparkling Waters.
According to the PSC HW guidance poster and documents, laundry detergent is to be placed in a Toxic HW tote.
Also, the safety data sheet(SDS)for this laundry detergent states that strong oxidizers are incompatible with the
detergent, and the SDS shows the GHS05 hazardous pictogram for corrosive. The SDS was reviewed after the date of
inspection.
3.The waste items listed below were found bagged and inside a tote labeled Toxic HW(IMG 7971, 7979-7982):
-A Revlon Color Silk permanent hair color kit, which contained a container of ammonia-free colorant, a container of
cream color developer, and a package of after-color conditioner. Hydrogen peroxide is listed as the second ingredient
in the developer.According to the PSC HW guidance poster and documents, items containing hydrogen peroxide are
to be placed in the Oxidizer HW tote. The hazardous properties of the ammonia-free colorant and the after-color
conditioner are unknown.
-A bag of Kingsford Original Charcoal Briquets.
A generator shall make a hazardous waste determination and keep a record of any test results, waste analyses, or
other determinations made in accordance with hazardous waste regulations for at least three years from the date that
the waste was last disposed of.
Immediately locate a copy of the hazardous waste determination made for the hazardous waste items listed above,
ensure store employees are familiar with these waste determinations, and submit a copy to the EHD.
If a copy is unavailable, immediately make a hazardous waste determination and begin managing the above listed
waste under Title 22 hazardous waste regulations. Submit a statement to the EHD documenting the hazardous waste
FA0001305 PR0537830 SCO01 07/31/2019
EHD 22-01 Rev.09/27/2018 Page 5 of 12 Small Quantity Hazardous Waste Generator OR
1868 E.
