Below I have compiled responses and attached backup documentation for your review. <br /> Feel free to contact me with further questions or requests I ' ll do my best to provide the necessary information. l <br /> Item #209 <br /> Observation 1 DO reports not all signed : The reports are reviewed monthly with the owner/operator, <br /> after clarifying section VI and moving forward each report will be delivered within the <br /> designated time period to the owner/operator. I do know the new forms are more specific <br /> regarding the time frame for delivery of reports and compliance should not be an issue . <br /> Observation 2 DO failed to check and note test dates : See related attachments after careful review <br /> Secondary containment date was inadvertently miss entered and replicated over a few <br /> months of reporting this has been corrected for all reports . Overfill prevention test was <br /> correct see attached. Overfill protection as indicated within the reports was originally <br /> completed 12/28/ 18 as you indicated in your notes however, it may have been an invalid <br /> after further review it was decided to perform another inspection on 3/25/ 19 .
