OBSERVATION: A monitoring system certification, leak detector testing, and spill container testing were performed on 4/19/2019. Atest report has not been submitted to the EHD or the California Environmental Reporting System (CERS). REGULATION GUIDANCE: A copy of the test report must be submitted within 30 days of the test. CORRECTIVE ACTION: Immediately provide this report to the EHD. This is a minor violation. 201 CCR 2712(b)(1-2) Failure to maintain monitoring, maintenance, and follow up action records. OBSERVATION: Maintenance and monitoring records for the last three years were not found on site. -Work orders for the removal of water/liquid from the 87 and 91 vapor and fill containers and diesel fill container on the following dates; 01/28/2020, 12/4/2019 and 4/15/2020 were not found on site. REGULATION GUIDANCE: These records shall be maintained on site for at least three years. Monitoring records include: (1) date and time of all monitoring or sampling; (2) monitoring equipment calibration and maintenance records; (3) results of any visual observations; (4) results of sample analysis performed a lab or in the field; (5) logs of all readings of gauges or other monitoring equipment, ground water elevations, or other test results; (6) results of inventory readings and reconciliations. CORRECTIVE ACTION: Immediately locate and maintain all missing maintenance and monitoring records for the last three years on site and submit copies to the EHD. This is a minor violation. 202 CCR 2715(c)(3), 2715(c)(4) Failure to maintain the "Facility Employee Training Certificate" on-site(after October 1, 2018). OBSERVATION: The Facility Employee Training Certificate dated 2/21/2020 and 4/15/2020 indicate that: - Rajbir Singh and Gagandeep Deol have not been trained. REGULATION GUIDANCE: All site employees must be trained prior to taking responsibility as a facility employee if after October 1, 2018, or within 30 days of hire if prior to October 1, 2018. The Facility Employee Training Certificate shall contain accurate information for all employees trained. CORRECTIVE ACTION: Update the Facility Employee Training Certificate to contain the accurate information and submit a copy of the updated form to the EHD. This is a minor violation.
