The following is an itemized list of underground storage tank violations that have not <br /> been addressed for AGS FUEL INC DBA CIRCLE K CHEVRON as of May11 <br /> 2020. <br /> Open violations from March 03, 2020 inspection <br /> ❑ This violation was corrected ❑ This violation will be corrected by (date): <br /> ❑ Supporting documents included <br /> Describe actions taken or will be taken to correct violation: <br /> Violation #209 - Failure to conduct the designated UST operator visual inspection at least once every 30 <br /> days. <br /> OBSERVATION Designated operator reports were not signed within the required time frame. <br /> -The 3/28/2019 designated operator report was signed by the owner/operator on 4/18/2019. <br /> -The 6/13/2019 designated operator report was signed by the owner/operator on 8/8/2019. <br /> -The 7/11/2019 designated operator report was signed by the owner/operator on 8/18/2019. <br /> REGULATORY GUIDANCE Within 48 hours of the completion of the designated UST operator visual inspection, <br /> the designated operator shall sign and provide the owner or operator with a copy of the inspection report. Within 48 <br /> hours of being provided a signed copy of the report, the owner or operator shall provide a description of each <br /> corrective action taken or to be taken and shall sign and date the report, acknowledging the identified compliance <br /> issues. <br /> CORRECTIVE ACTION: Ensure all designated operator inspection reports are completed and signed within the <br /> required time frame in the future. <br /> ❑ This violation was corrected ❑ This violation will be corrected by (date): <br /> ❑ Supporting documents included <br /> Describe actions taken or will be taken to correct violation: <br /> Violation#313 - Failure to construct, operate, and maintain primary containment as product-tight. <br /> OBSERVATION Approximately 1 cup of liquid was found in UDC 5/6 and UDC 7/8, indicating a leak in the primary <br /> containment. The diesel meter in UDC 7/8 was actively leaking at the time of the inspection. <br /> REGULATION GUIDANCE: All primary containment for the UST system must be product tight. <br /> CORRECTIVE ACTION: Immediately have a properly licensed, trained, and certified contractor repair or replace the <br /> failed component(s), under permit and inspection of the EHD if necessary. <br /> Page 3 of 4 <br />
