The permit has been <br /> approved with the following conditions.- <br /> 1. <br /> onditions:1. Contact the local fire department and the San Joaquin Valley Air Pollution Control District for <br /> any applicable permits and requirements. <br /> 2. Any changes in information and scope of work must be submitted to EHD for approval prior to <br /> commencing work. All new addenda supersede the previous ones. <br /> 3. Contact John Alaniz at (209) 468-3147 to arrange the following inspection appointments (a 48- <br /> hour notice is required for inspection appointments). Please be aware that inspections for <br /> emergency work must be scheduled within five days of permit approval. <br /> a. An ISD upgrade is going to be occurring as part of the service request. If it is <br /> determined that a cold start is necessary, a copy of the alarm history shall be printed <br /> prior to performing the "cold start" of the Veeder Root system and made available at the <br /> time of EHD inspection. Functionality of the monitoring system must be verified to <br /> ensure that monitoring has been restored to meet the requirements of Title 23 California <br /> Code of Regulations and the Health & Safety Code. <br /> b. Leak detector test: The new Diesel leak detector must be tested to confirm that the <br /> device can detect a leak of 3 gallons per hour at 10 psi. <br /> c. Verification that all potential dispenser piping leaks will be contained by the under <br /> dispenser containment. Prior to this inspection, send in the Bravo Dispenser <br /> Conversion Evaluation Form to the EHD. See the permit packet for the form. <br /> d. Visual inspection of piping within all of the dispensers replaced. <br /> e. Witness secondary containment testing on the diesel turbine sump and the 1/2 , 5/6 <br /> under dispenser containers <br /> Please be aware that the facility cannot operate the UST system or receive fuel in the <br /> Diesel tank prior to the EHD inspection and approval. <br /> 4. Provide hazardous waste manifest for tank and piping rinsate within 180 days of waste <br /> generation. <br /> 5.
