Date: 08/30/2019 <br /> Manufacturer training: INCON, 06/04/20 <br /> OVERALL INSPECTION NOTES AND COMMENTS <br /> NOTES <br /> On site to witness the following: <br /> -A 0.002 gallon per hour leak rate using the INCON system for 15 minutes in UDC 5/6 and UDC 7/8. Both UDC passed.This <br /> test was performed by service technician Juan J.Jauregui ICC certifiction expiration date 8/30/19, INCON certification <br /> expiration 6/4/19 <br /> -Passing results for the Diesel tank to confirm that the device can detect a leak of 3 gallons per hour at 10 psi.This testing <br /> was performed by service technician Nicholas Joane ICC certification expiration date 4/11/19 and VMI certification expiration <br /> 07/13/19. <br /> This Service request is NOT completed the following need to be addressed: <br /> -Hazardous waste manifest for tank and piping reinstate within 180 days of waste generated must be provided. <br /> -Tank owner or operator shall log into the California Environmental Reporting System(CERS) at http://cers.calepa.ca.gov/all <br /> pertinent forms and update and submit the"UST monitoring plan"for this tank conversion.FURTHERMORE while the <br /> inspector was on site, overfill prevention equipment inspection was conducted for all three tanks.The integrity of the Diesel <br /> product tank spill container was compromised during the testing.After performing a 60 minute hydrostatic test the Diesel <br /> product direct-bury spill container failed when tested.All spill containers shall have a minimum capacity of five gallons and be <br /> capable of containing a spill or overfill until it is detected or cleaned up. IMMEDIATELY DISCONTINUE deposition of <br /> petroleum into this tank until the component is repaired or replaced by a properly licensed,trained,and certified contractor <br /> under permit and inspection of the EHD. The Diesel product fuel levels are at 4738 gallons.
