UST Tank Information forms for all three tanks are not current in CERS. -On all three tanks under Vent,Vapor Recovery(VR)and Riser/Fill Pipe Piping Constructio it states primary containment to be steel and secondary containment for vent to be fiberglass. -For the 87 product tanks, it states that Vapor recovery primary is steel and secondary as fiberglass however single wall fiberglass vapor recovery was pipes were observed during the inspection. Any change of information must be updated in CERS within 30 days of the changes. Immediately log into CERS, update the required information,and submit for review by the EHD. This is a repeat violation, Class II. 105 CCR 2632(d)(1),2634(d) Failed to have an approved UST monitoring plan. The monitoring plan is not current and/or not approved by the EHD. Under record keeping it list"Tank Integrity Testing Results"however in the DO reports it states that the facility does not perform tank integrity testing. The monitoring plan must be uploaded to the California Environmental Reporting System(CERS). Immediately log into CERS, make the necessary changes,and submit for review by the EHD. This is a minor violation. 113 CCR 2638(c),2638(d) Failure to submit"Monitoring System Certification Form"within 30 days of completion of the test. Annual monitoring system certification, leak detector testing,and spill container testing was performed on 10/09/17 and a test report was submitted on 4/24/18,five months late. A copy of the test report must be submitted within 30 days of the tests. Take all necessary precautions to ensure testing and submittal of test reports are performed in a timely manner. This is a Class 11 violation. 121 CCR 2712(f) Failed to submit a written response within 30 days of receiving an inspection report. An inspection was last done on 10/09/17 and an inspection report was issued identifying information to be submitted to bring this site into compliance. This information was required to be submitted by 11/08/17.This information was received on 5/31/18,6 months late, resulting in a non-compliant status for this facility. An operator that receives an inspection report shall have 30 days to submit a written response that includes a statement documenting corrective actions taken or proposing corrective actions which will be taken. Ensure that a written response documenting corrective actions taken or proposed is submitted within 30 days of receiving an inspection report. This is a repeat violation,Class It.
