Tracy Blvd. <br /> Tracy, CA. 95304 <br /> The San Joaquin County Environmental Health Department (EHD) has completed the review of the <br /> application for a permit to remove and replace the 91 submersible turbine sump (STP) entry fitting with <br /> a Diversified Products Manufacturing, Inc split repair entry fitting on the underground storage tank <br /> (UST) system at the above noted address. The permit has been approved with the following <br /> conditions: <br /> 1. Contact the local fire department and the San Joaquin Valley Air Pollution Control District for <br /> any applicable permits and requirements. <br /> 2. Any changes in information and scope of work must be submitted to EHD for approval prior to <br /> commencing work. All new addenda supersede the previous ones. <br /> 3. Contact Betty Ho at (209) 468-3455 to arrange the following inspection appointments (a 48- <br /> hour notice is required for inspection appointments). Please be aware that inspections for <br /> emergency work must be scheduled within five days of permit approval. <br /> a. Secondary containment testing on the impacted 91 STP sump. <br /> Please be aware that the facility cannot operate the UST system tank prior to the EHD <br /> inspection and approval. <br /> 4. Secondary containment testing for the impacted 91 STP sump is required six months after <br /> installation. Provide 48 hours notification to the EHD when this test is scheduled. <br /> 5. All testing conducted must have passing results for the repaired sections to be allowed to <br /> operate. If repairs are done in phases, all testing associated with a particular phase must have <br /> passing results before section is allowed to operate. <br /> 6. Any individual performing the work of a service technician must meet the following <br /> qualifications: <br /> a. Possess or be employed by a person who possesses one of several permissible valid, <br /> current contractor's licenses issued by the Contractors State License Board (may be issued <br /> to the installation company), or a Tank Tester's license issued to the individual by the State <br /> Water Board, and <br /> 1868 E.
