Subsequently a Problem Assessment <br /> Report (PAR) was submitted which indicated groundwater <br /> contamination of the site. Please submit an additional copy of the <br /> PAR to PHS/EHD and to the Central Valley Regional Water Quality <br /> Control Board (CVRWQCB) . <br /> The extent of groundwater contamination and the groundwater <br /> gradient have not been confirmed at this time. Therefore, <br /> additional monitoring wells will be necessary to provide this <br /> information. <br /> Pursuant to Section 2652 (d) of the Underground Storage Tank <br /> Regulations (California Code of Regulations, Title 23 , Subchapter <br /> 16) a quarterly monitoring report shall be submitted to FHS/EHD and <br /> CVRWQCB until the investigation and cleanup of the site is deemed <br /> adequate. The report is to show the: <br /> -status of investigation and cleanup activities including <br /> the results of all investigations completed to date; <br /> -proposed continuing or next phase of investigation; <br /> -method of cleanup proposed or implemented, schedules for <br /> the completion of the investigation of the site and <br /> remediation; <br /> -method and location of disposal of the released <br /> hazardous substance and any contaminated soil, <br /> groundwater or surface water; <br /> A Division of San Joaquin County Health Care tierces <br />
