To obtain a copy, please contact the SWRCB at(916) 657-2390. Remedial
Action Completion certification occurs only after PHS/EHD has determined that no further
site investigation or remedial action is required.
In order to determine if the contaminants detected during the October 15, 1996 underground tank
removal have impacted the groundwater beneath the site, a workplan for further investigation is
now required. The proposal should address the lateral and vertical limits of the soil contamination
and should include gathering and analyzing soil and groundwater samples. This plan should be
submitted to PHS-EHD for review by June 2, 1997. Please include a site safety plan with your
workplan submittal.
All work and reports which require geological or engineering evaluations must be performed under
the direction of an appropriately California registered or certified professional.
y If you have any questions or there are any corrections in the responsible party information,
contact Michael Infurna at(209)468-3454.
Donna Heran, REHS, Director
Environmental
Health Division
Michael Infurna Jr., Senior REHS ZrgarleLagorio, REHS
a
LOP/Site Mitigation unit IV Supervisor
MI:ib
c: SWRCB, Underground Storage Tank Section-Lori Casias
c: RWQCB, Central Valley Region -Elizabeth Thayer
A Division of San Joaquin County Health Care Services
