This facility does not have a valid EPA ID number to manage hazardous waste. A hazardous waste generator shall not treat, store, dispose of, transport or offer for transportation, hazardous waste without an EPA ID number. Immediately obtain an EPA ID number to manage hazardous waste and submit evidence to the EHD. This is a Class II violation. 102 CCR 66262.11, 66262.40(c) Failed to determine if a waste is a hazardous waste or retain waste analysis on site for 3 years. Currently, -all expired, creditable pharmaceuticals are being sent via Reverse Distribution to EXP without a waste determination being made. -all dropped, and or broken pills are being placed in a blue box on the wall that is being sent to the hospital for disposal without a waste determination being made. Any person who generates a waste shall determine if the waste is a hazardous waste. Immediately make a hazardous waste determination for the above wastes, and manage it according the Title 22 hazardous waste regulations. Submit a statement and supporting documentation explaining how this waste was managed. This is a Class II violation.
