Beginning January 1, 2013, all businesses are required to submit all new(or any changes to existing) <br /> hazardous waste information online to the CERS at http://cers.calepa.ca.gov. Be sure to include your hazardous <br /> waste activity in the Businesses Activities section in CERS in addition to any other relevant activities and required <br /> fields. <br /> This is a Class II violation. <br /> 202 HSC 25189.5(a) Disposed or caused disposal of HW at an unauthorized point. <br /> According to Mai Trinh, Outpatient pharmacy manager, she had not received hazardous waste training. She stated <br /> she would throw Coumadin and Warfarin (P-listed hazardous waste)containers in the trash. She was not aware of <br /> RCRA or NON-RCRA wastes. There was a"Proper Waste and Linen Segregation & Disposal by Container Type" <br /> chart which listed the container hazardous waste should be placed in, above the blue disposal container, that was <br /> being used for dropped and broken pills. Hazardous wastes shall be disposed of only by transportation to a permitted <br /> hazardous waste treatment, storage, and disposal facility (TSDF). Immediately cease disposal of hazardous waste to <br /> the trash and ensure that all future waste is hauled by a licensed hazardous waste transporter to a permitted TSDF. <br /> This is a Class I violation. <br /> 203 HSC 25189.5(c) Transported or caused transportation of hazardous waste to an unauthorized point. <br /> Hazardous waste was transported for storage and ultimate disposal to 525 W. Acacia St., Stockton from this address. <br /> 525 W. Acacia St., Stockton CA is not a permitted hazardous waste treatment, storage, and/or disposal facility <br /> (TSDF). Hazardous waste shall be disposed of only by transportation to a permitted TSDF. Immediately cease <br /> transportation and disposal of hazardous waste at an un-permitted facility and ensure that all future hazardous waste <br /> is hauled by a licensed hazardous waste transporter to a permitted TSDF. <br /> This is a Class II violation. <br /> Overall Inspection Comments: <br /> Checklist on violations left on site on March 31, 2015. <br /> NOTE: Violations 106 and 203 were added for the transportation of dropped/broken/loose pills to Dameron <br /> Hospital for disposal. <br /> Official Inspection report to be mailed certified April 27, 2015. <br /> Complete and submit a completed copy of the Return to Compliance Certification with a statement addressing <br /> how each of the above listed violations has or will be corrected to this office within 30 days of receipt of the full <br /> written official inspection report.
