MARKET STREET; STOCKTON <br /> HYDROCARBON EVALUATION STUDY DATED APRIL 26, 1988 <br /> We are in receipt of the above referenced environmental review prepared by <br /> Balbi & Chang Associates. The Boring Logs presented within this report do <br /> not identify the contractor. Installation of borings and/or monitoring <br /> wells within the county of San Joaquin require a C-57 license, proof of <br /> workmen's compensation insurance, and a permit issued through the San <br /> Joaquin Local Health District, Environmental Health Division. <br /> Insomuch as this environmental review was conducted without benefit of the <br /> above local permit requirements, we are requesting the contractor submit a <br /> completed application form, provide the necessary license and insurance <br /> documents, and the permit fee of $35.00 within 15 days from the date of this <br /> letter. Failure to do so may result in administrative and/or legal action <br /> by this department. <br /> The issue of the tank(s) removal at this location is still unclear. <br /> Registration and closure fees were submitted to this department January 5, <br /> 1988; however, actual closure details are not available. Please provide <br /> details as to the date of the tank(s) removal and the contractor who <br /> performed the closure activities. This information can be included in your <br /> response to us regarding the drilling contractor. <br /> In addition to the above and pursuant to the San Joaquin Local Health <br /> District fee schedule Resolution No. 85-58, a $35.00 consultation fee shall <br /> be submitted along with the completed general Environmental Health <br /> application form for review of the above referenced environmental <br /> assessment/remediation activity. <br /> Each additional hour over the initial one hour of time spent in review <br /> activity will be billed at the rate of $35.00 per hour. The additional <br /> amount due will be billed to the applicant identified on the general <br /> Environmental Health application format the completion of requested <br /> activity, or monthly, if longer than 30 days is required for completion. <br />
