I have included a copy of the"Cost Pre- <br /> Approval Request"form; please use this form in the future for requesting pre-approval of corrective <br /> actions costs. <br /> With the following provisions,the total cost pre-approved as eligible for reimbursement for three months <br /> of thermox rental and preventive startup and maintenance approved by the San Joaquin County EHD <br /> (County)per a telephone conversation with Mr. Ron Rowe is $2670. This includes thermox rental at a <br /> reduced standby mode rate of$550/month and maintenance at$340/month from April thm June. It is my <br /> understanding that the system has been in the standby mode since January awaiting a decision from the <br /> regulatory agencies. It is the Fund's position that cleanup should proceed in a cost effective and timely <br /> manner. Since maintenance and rental costs are still incurred during the system standby mode without the <br /> site undergoing active remediation, lengthy periods of system inoperation could result in higher cleanup <br /> costs. It is imperative that a determination be made within the next month or sooner as to what the next <br /> course of action will be to prevent incurring higher cleanup costs. <br /> Your pre-approval request includes costs from January thm June.Please be reminded that pre-approval of <br /> costs only includes future costs and therefore pre-approval of the January thru March costs which totals <br /> $5090 is inappropriate. You may submit these incurred costs on your next reimbursement request. The <br /> total amount approved for payment through request number 12.0 for work at your site that has been <br /> directed and approved by the County is $ 395,950.) <br /> Be aware that this pre-approval does not constitute a decision on reimbursement: all reasonable and <br /> necessary corrective action costs for work directed and approved by the County will be eligible for <br /> reimbursement per the terms of your Letter of Commitment at costs consistent with those pre-approved in <br /> this letter. <br /> All future costs for corrective action must be approved in writing by Fund staff. <br /> Future costs for corrective action must meet the requirements of <br /> Article 11, Chapter 16, Underground Storage Tank Regulations. <br /> • The actual costs and scope of work performed must be consistent with the pre-approval for it to <br /> remain valid. <br /> California Enviroamenta/Protection Agency <br /> A <br /> �a Recycte,IPaper <br />
