Box 1257 <br /> San Ramon, CA 94583 <br /> Subject: ARCO Station #5469 <br /> 130 South Wilson Way <br /> Stockton, CA 95205 <br /> The San Joaquin County Environmental Health Department (EHD) has reviewed Work Plan for <br /> Well Destruction and System Decommissioning (Work Plan) dates{ January 10, 2012, submitted <br /> on your behalf by Arcadis, and has the following comments. <br /> In the Work Plan Arcadis outlines the destruction of all groundwater monitoring and remediation <br /> wells associated with this site. Three wells, MW-2, MW-3 and MW-7 are proposed to be <br /> destroyed by pressure grouting with neat cement grout; the remaining groundwater and <br /> remediation wells are proposed to be destroyed by overdrilling the entire well boring and filling it <br /> with neat cement grout. <br /> The EHD approves the well destructions outlined in the Work Plan with the condition that after <br /> application of the grout seal the top three feet of the borings, or two feet for the wells destroyed <br /> by pressure grouting, are filled with clean soil to just below surface grade, which will then be <br /> completed to match existing surface conditions at each location. <br /> Please proceed with submittal of a San Joaquin County Unit IV Well Permit Application, <br /> $375.00 inspection fee and $125.00 per well destruction permit fee for each parcel of land on <br /> which work will be completed, and a signed Unit IV Site Mitigation Master File Record Form. <br /> If you have any questions please contact Nuel Henderson at (209) 468.3436 or by email at <br /> nhenderson s'cehd.com. <br /> Lori Duncan, REHS Nuel C.
