SAN JOAQUIN <br />c <br />Environmental Health Department <br />AMENDED <br />11125/2019 <br />Small Quantity Hazardous Waste Generator Inspection Report <br />Facility Name: <br />GOLDEN STATE LUMBER INC <br />Facility Address: <br />3033 S Airport Way, Stockton <br />Date: <br />November 22, 2019 <br />SUMMARY OF VIOLATIONS <br />(CLASS I, CLASS II, or MINOR - Notice to Comply) <br />Item # Remarks <br />1 1 3 HSC 25160.2(b)(3) Failed to keep copies of consolidated manifesting receipts for three years. <br />OBSERVATION: Department of Toxic Substance Control records indicate that hazardous waste was hauled away <br />on the following consolidated manifests: <br />-12/20/2016 (Aqueous Solution, 55 gallons) <br />-12/20/2016 (Used Oil, 360 gallons) <br />-2/2/2017 (Used Oil, 205 gallons) <br />-3/20/2017 (Used Oil, 225 gallons) <br />-4/18/2017 (Used Oil, 200 gallons) <br />-6/26/2017 (Used Oil, 290 gallons) <br />-8/18/2017 (Used Oil, 175 gallons) <br />-10/3/2017 (Used Oil, 110 gallons) <br />-12/5/2017 (Used Oil, 335 gallons) <br />The consolidated manifests listed above were not found on site during the inspection. <br />REGULATION GUIDANCE: The generator shall retain each consolidated manifest receipt for at least three years. <br />This period of retention is extended automatically during the course of any unresolved enforcement action regarding <br />the regulated activity or as requested by the department or a certified unified program agency. <br />CORRECTIVE ACTION: Immediately locate a copy of the missing consolidated manifest receipts from above and <br />maintain them on site.
