Mr Jeffrey Wong <br /> i 'f 11 Page 3 of 10 Tank #I excavation were described as having severe odor and discoloration from the surfa <br /> the sampling depth of 10 feet ce to <br /> PHS/EHD directed the responsible party to conduct a subsurface investigation in a letter dated <br /> August 16, 1999 Ground Zero prepared a workplan dated October 13, 1999 on behalf of the <br /> Ferraiolos and submitted the report to PHS/EHD for approval PHS/EHD approved the workplan <br /> on November 30, 1999 <br /> 2.3 Initial Soil Boring and Monitoring Well Investigation <br /> On February 22 and 23, 2000, one monitoring well and eight soil borings were advanced at the <br /> site to Investigate the extent of contamination <br /> Borings SB 1 and SB2 were drilled in the vicinity of the former tank pit SB I was converted Into <br /> groundwater monitoring well MW1 Barings SB3, SB3a through SB3c, S134 and SB5 were located <br /> peripheral to the former tank excavation The locations of the borings and the monitoring well are <br /> shown on Figure 2 <br /> Soil samples collected from borings SBI and SB2 indicated detectable concentrations of gasoline <br /> hydrocarbons to SO feet below ground surface (bgs) The remaining borings had no detectable <br /> amounts for the target analytes No oxygenated additives were detected in any soil sample Soil <br /> analytical results are summarized in Table 2 <br /> Groundwater sampled from well MW 1 contained <br /> relatively high <br /> ntratialareultfdBTEX No oxygenate additives were detected in roundwatersample � yticl s <br /> groundwater are summarized in Table 3 <br /> The results of the investigation were reported by Ground Zero in the Subs-urface Investigation <br /> Report dated April 12, 2000 PHS/EHD responded to the report in a letter dated July 24 in which a <br /> workplan for additional investigation was required <br /> Ground Zero prepared the Workplan for Additional Subsurface Investigation slated October 18, <br /> 2000 PSD approved the workplan in a letter dated November 30, 2000 <br /> 3.0 INVESTIGATIVE PROCEDURES <br /> In accordance with the approved workplan, CPT and soil borings were advanced and groundwater <br /> monitoring wells were installed to further evaluate the nature and extent of soil and groundwater <br /> impact <br /> 3.1 Permitting <br /> Prior to initiating fieldwork, well installation permits were obtained from PHS/EHD All fieldwork <br /> was performed in accordance with the Ground Zero's Site Safety Plan and Quality <br /> Assurance/Quality Control Plan <br /> datllgrounzelhlrryslwellreport doe <br />
