Please provide the following additional information (three copies each) and <br /> address the following comments: <br /> 1) Page 3 of the Application for Underground Storage Tank Installation Permit: Indicate the <br /> Contractor's name, phone, address, license number, hazardous waste certificate, <br /> worker's comp, fire district permit number, and UST Board of Equalization number. Tank <br /> ID 2b indicates unleaded gasoline as the chemical stored. Please change to diesel. <br /> 2) Page 4, Section F of the permit application: Provide accurate piping model numbers for <br /> all piping being used, including the use of multiple sizes of vapor piping (if applicable). <br /> 3) Page 5, Section G of the permit application: Identify which overfill prevention method will <br /> be utilized at this facility. Provide accurate manufacturer and model information. If a <br /> mechanical flapper will be utilized, drop tube measurement calculations will need to be <br /> provided prior to install. <br /> 4) Page 6, Section I of the permit application: The tank annular sensor model number listed <br /> is not intended for use in a wet annular. x6liKinder dispenser containment sensor is <br /> listed as N/A. Provide accurate sensor model numbers. <br /> 5) Provide current contractor's training certifications for installation and testing of all <br /> proposed UST equipment/components. <br /> 6) Provide manufacturer's cut sheets for all proposed UST system components, including <br /> but not limited to: under dispenser containment, turbine (STP) sumps, fill sumps, vent <br /> sumps, transition sumps, vacuum sensors, annular sensors, electrical penetrations, <br /> sump hydrostatic fluid, and overfill prevention. <br /> 7) Provide tank charts for the tanks being used. <br /> 8) Submit a work plan for enhanced leak detection (ELD) testing, to be conducted by a <br /> properly licensed individual. <br /> 9) Provide a diagram identifying the vacuum zone volumes and smart sensors that will be <br /> used for piping monitoring. <br /> 10)Provide a drawing indicating the locations of all water wells, septic systems, and storm <br /> drains. <br /> I <br />
