CLEANUP AND ABATEMENT ORDER NO R5-2003-0713 9 <br /> FRANK and SHERRI GUINTA, dba GUINTA ENTERPRISES <br /> JAMES and MARILYN RAMSEY, <br /> 2072 WEST YOSEMITE AVENUE <br /> MANTECA, SAN JOAQUIN COUNTY <br /> General Requirements <br /> 18 Conduct investigative and remedial work only upon approval of work plans by Regional Board <br /> staff <br /> 19 As required by the California Business and Professions Code Sections 6735 7835 and 7835 1, <br /> have all reports prepared by, or under the supervision of, a registered professional engineer or <br /> geologist and signed by the registered professional All technical reports submitted by the <br /> Dischargers shall include a statement signed by the Dischargers certifying under penalty of law that <br />' the Dischargers have examined and are familiar with the report and that to Dischargers' <br /> knowledge, the report is true, complete, and accurate <br />' 20 The Dischargers shall routinely optimize remedial systems to reduce the time to achieve cleanup <br /> objectives The effectiveness of the optimization efforts shall be reported in the Quarterly <br /> Monitoring Reports <br /> 21 The Dischargers shall notify Regional Board staff at least three working daysan rior to onsite <br /> p Y <br /> work, including quarterly monitoring events, dnlling, surveying, and instal lation/modification of <br /> 10 remedial systems This notification is not required for routine maintenance and monitoring of <br /> remedial systems <br /> 22 Obtain all local and state permits and access agreements necessary to fulfill the requirements of <br /> this Order pnor to beginning the work Some permits and access agreements may require several <br /> months to obtain, so the Discharger must submit permit applications and initiate access <br /> fnegotiations in a timely manner to assure compliance with this Order <br />' 23 Continue any remediation or monitoring activities, including monitoring and treatment of private <br /> domestic wells, until such time as the Executive Officer determines that sufficient cleanup has <br /> been accomplished and this Order has been rescinded <br />' 24 Analytical results shall be submitted electronically using the Electronic Deliverable Format (EDF) <br /> for analyses of soil and/or groundwater samples, in addition to the customary paper format In <br />' addition to the laboratory data, site specific information is required to be submitted in electronic <br /> format for the following 1) the latitude and longitude of groundwater monitoring wells (including <br /> any other well(s) designated as part of the site monitoring program) accurate to within one meter, <br />' 2) the surveyed elevation, relative to mean sea level, for any groundwater sampled, accurate to <br /> within a tenth of a foot, 3) groundwater information, including depth to water, free product <br /> presence/thickness and well status, and 4) a site map in electronic format (PDF,JIFF, TIFF, JPEG, <br /> etc ) showing property boundanes, buildings, and soil and water sampling locations All EDF data <br /> must be submitted over the Internet to the State Water Board Geographic Environmental <br /> .
