The MW-UST is to be destroyed using a hollow stem auger to over drilled and remove the <br /> casing. The augers will then be used as a tremmie to backfilled with neat cement. <br /> 2. The traffic box which was installated at the surface will be removed. <br /> 3. All cutting that are generated during over drilling will be containerized, characterized, <br /> labeled, and properly disposed of. <br /> { <br /> COMMENTS AND CONDITIONS: <br /> 1. No EHD directive or permit was issued for the installation of MW-UST. No EHD inspection <br /> was performed during installation and no construction reports are on file. The MW was <br /> installed sometime after over excavation of contaminated soil by AGE from the former UST <br /> location near San Jose Street. <br /> 2.
